Joseph Selaby has moved from the town to the country - from a London hospital to a practice in a small rural town. He buys a notebook and starts jotting down his experiences, past, present and possibly future. Observations, anecdotes and portraits of his patients are also 'noted', and thus the notebook grows to offer a kaleidoscopic view of well-being and ill-health and the varying physical and mental states in-between. Selaby's notes run the gamut of emotions both personally and through the people he treats and tries to cure. We come to know the doctor as an obstinate idealist and a moral man, and in time he reviews an old love affair and embarks with a mixture of courage and caution on a new one.
